Normal CAC & ID Cards Issuance

As of 15 April, nationwide connectivity to DEERS/RAPIDs has been restored. 6 FSS is now able to resume normal CACs/dependent IDs support, update to DEERS records, and accomplish CAC PIN resets. Customers with non-urgent CAC/DEERS issues are strongly encouraged to schedule an appointment prior to visiting the MPS; appointments can be scheduled by visiting:

Military Priority in Customer Service:

For your convenience, Customer Service offers military priority M-F, 0730-0930. This is for the military member only, and is used to expedite the member back to their unit. If you are unable to accompany your dependent(s) at another time, we will be happy to issue a DD Form 1172-2, at that time, to be given to your dependents. They may then come during our walk-in hours, or make an appointment at:

INSTRUCTIONS: Click "Make Appointment" and select "FL" from the drop down list, then click "Go". Choose "Tampa" from the drop down list and click "Next". You will then see our address, phone number and a Google map. Click "Select". On the calendar, click on the day you want to make your appointment, then select an available time. Enter information in all fields under "Your Information," then click "Submit".

  • Entered into DEERS via CVS (Contractor’s Verification System)
  • Two valid forms of identification

Spouse and Children (10 to 20 years of age)

  • Sponsor (Military or Retired member) or Power of Attorney (General/Special) or DD Form 1172 notarized
  • Two valid forms of identification

Children (Over 21 years of age)

  • Sponsor (Military or Retired member) or Power of Attorney
  • Letter from school Registrar stating full time and anticipated graduation date
  • Two forms of identification

100% DISABLED VETERAN ID CARDS:

  • DD Form 214
  • VA Letter stating that the honorably discharged veteran is 100-percent disabled from a service-connected injury or disease; whether evaluation is necessary, and that commissary and exchange are authorized
  • Marriage License (if getting a Dependent ID card)

  • An unexpired Employment Authorization Document that contains a photograph (Form I-766, I-688, I-688Am U-688B)
  • An unexpired foreign passport with an unexpired Arrival-Departure record, Form I-94
  • Driver’s license or ID card issued by a state
  • ID card issued by federal, state or local government agencies or entities
  • School ID card with a photograph
  • Voter’s registration card
