CAC/ID Card Issuance Restored

As of 17 Sept 14, nationwide connectivity to DEERS/RAPIDs has been restored. 6 FSS is now able to resume normal CACs/dependent IDs support, update to DEERS records, and accomplish CAC PIN resets. Customers with non-urgent CAC/DEERS issues are strongly encouraged to schedule an appointment prior to visiting the MPS; appointments can be scheduled by visiting

New Limited Walk-In Service

Starting Monday, 1 September, Customer Service implemented a Limited Walk-In Service.
Walk-Ins will only be accepted Monday, 0730-1100 & Tuesday-Friday, 0730-0930.

Military Priority in Customer Service:

For your convenience, Customer Service offers military priority M-F, 0730-0930. This is for the military member only, and is used to expedite the member back to their unit. If you are unable to accompany your dependent(s) at another time, we will be happy to issue a DD Form 1172-2, at that time, to be given to your dependents.

Enter information in all fields under "Your Information," then click "Submit". If all dates for the current month are in gray, this indicates there are no more available appointments. We open appointments for the following month, between the 18th and the 22nd.

INITIAL CAC ISSUE:

Military IDs (Geneva Convention Identification Card)

  • Entered into DEERS from military service tape
  • Two valid forms of identification

Civilian CAC

  • Entered into DEERS via the DCPDS from Civilian Personnel
  • Two valid forms of identification

Contractor’s CAC

  • Entered into DEERS via CVS (Contractor’s Verification System)
  • Two valid forms of identification

Newborns

  • Sponsor (Military or Retired member)

  • Two valid forms of identification if over 21 years of age

Wards and Foster Children

  • Dependency Determination/Approval from servicing DFAS
  • Birth Certificate

RENEWAL OF DEPENDENT ID CARDS:

Spouse and Children (10 to 20 years of age)

  • Sponsor (Military or Retired member) or Power of Attorney (General/Special) or DD Form 1172 notarized
  • Two valid forms of identification

Children (Over 21 years of age)

  • Sponsor (Military or Retired member) or Power of Attorney
  • Letter from school Registrar stating full time and anticipated graduation date
  • Two forms of identification

100% DISABLED VETERAN ID CARDS:

  • DD Form 214
  • VA Letter stating that the honorably discharged veteran is 100-percent disabled from a service-connected injury or disease; whether evaluation is necessary, and that commissary and exchange are authorized
  • Marriage License (if getting a Dependent ID card)

  • US Passport (Unexpired)
  • Permanent Resident Card/Alien Registration Receipt Card (Form I-551)
  • An unexpired foreign passport with a temporary I-1551 stamp
  • An unexpired Employment Authorization Document that contains a photograph (Form I-766, I-688, I-688Am U-688B)
  • An unexpired foreign passport with an unexpired Arrival-Departure record, Form I-94
  • Driver’s license or ID card issued by a state (unexpired)
  • ID card issued by federal, state or local government agencies or entities (unexpired)
  • School ID card with a photograph
  • Voter’s registration card
  • U.S. Military ID Card or Military dependent’s ID Card
  • U. S. Coast Guard Merchant Mariner Card
  • Native American Tribal document
  • Driver’s license issued by a Canadian government authority
  • U.S. Social Security Card
